BUCCAL FAT REDUCTION
Sharpen your features, naturally.
“Buccal fat removal isn’t for everyone — but in the right face, it reveals structure that was always there. It’s subtle sculpting with lifelong payoff.”
What is a Buccal Fat Reduction?
“After years of operating on the head and neck, I’ve developed an eye for facial harmony. A facelift isn’t just about pulling skin — it’s about resetting the clock with grace and preserving what makes your face uniquely yours.”
Buccal fat removal is an in-clinic procedure designed to remove fat from the buccal fat pad — a naturally-occurring pocket of fat found deep in were the lower cheeks are found. Some individuals have larger buccal fat pads due only to their genetics and though they do not go away with time, weight loss or exercise. Removing a small amount of buccal fat pads is conservative and with the careful evaluation of Sydney’s acclaimed Cosmetic surgeon, Dr. Mahadik, can provide better cheek bones and a more streamlined position to their features - this can provide a more defined and elegant anatomy.
At Esmee, Dr. Parag Mahadik performs the procedure through a small incision in your mouth. There are no visible scars, no need for general anaesthesia, and downtime is quite minimal. The goal is not to create hollow cheeks but to fine tune your features and bring out the best of your underlying bone structure.

BENEFITS OF A BUCCAL FAT REDUCTION
Goal: Reveal the angles of your face, reduce unnecessary fat/fullness, and bring balance to your natural
features.
Many patients report feeling more confident in both casual and formal situations because of the results -
whether that is a selfie, an occasion, or simply catching your reflection unintentionally. It’s one of those
quiet statements of confidence; confidence so evident it embodies in your face and in your style.
- Feel more confident in photos — no more hiding your face or relying on filters to define your jawline
- Look more like how you feel inside — lean, healthy, and in control
- Enhance your cheekbones and facial contour — so your features can shine
- Create a more defined lower face that aligns with the rest of your face and body
- Permanent results with no external scars
YOUR BUCCAL FAT REDUCTION PROCEDURE
Buccal fat removal will be done in-clinic with local anaesthetic. You will arrive and leave after the surgery on the same day. After Dr. Parag Mahadik numbs the inside of your mouth, he will make a small incision next to your upper molars to access the buccal fat pad.
Once access is achieved, a pre-measured amount of the fat pad will be teased out and removed, taking care not to over-reduce. The incision will be sealed with dissolvable sutures —there is no need for external stitches. The whole process typically takes approximately 30 - 45 minutes. After the surgery you’ll be able to talk, eat soft foods and go home.
RECOVERY AND SIDE EFFECTS
Buccal fat removal is low-risk but, like any surgery, must be performed by a skilled practitioner to avoid overcorrection or nerve injury.
- Day 1–2: Swelling and tightness; soft foods recommended
- Day 3–7: Bruising settles; most patients return to work
- Week 2: Significant reduction in swelling; you’ll see early contouring
- Week 4–6: Final results become visible as tissues settle
Most people feel fully comfortable in under a week, and results are considered permanent — since the removed fat does not regenerate.
YOUR BUCCAL FAT REDUCTION RESULTS
Results from buccal fat removal comes in a gradual process. You'll see more definition around your cheekbones, a slimmer profile, and an overall more contoured midface as your swelling diminishes
This is not a transformation that yells "surgery" - it is a transformation that discreetly shows off your best angles and allows you to be more confident in photographs, conversations, and daily life. Your makeup will go on smoother, your natural bone structure will be more noticeable and your face will look slimmer and younger. The result is permanent, and there is no need for 'top ups' or 'maintenance' down the road.
