NIPPLE AND AREOLA SURGERY

Reconstructing completeness.

“This procedure is deeply symbolic. It’s the final touch in reclaiming the body — and often the confidence — after breast reconstruction.”

- Dr. Parag Mahadik

What is Nipple and Areola Correction?

These are minor surgical procedures designed to address specific concerns with the nipples or areolas. Some of the most common issues include inverted or retracted nipples, enlarged nipples, enlarged areolas (often post-pregnancy), puffy or dome-shaped areolas, and asymmetry between nipples or areolas.

Depending on your unique anatomy and goals, at Esmee, our renowned Sydney’s aestehtic surgeon Dr. Mahadik will recommend the best technique to enhance symmetry, proportion, and even functionality—such as improving sensitivity or making breastfeeding easier. Every effort is made to ensure scars are discreetly hidden along the natural borders of the nipple and areola, so they blend seamlessly into your skin.

AM I A GOOD CANDIDATE FOR NIPPLE AND AREOLA SURGERY?

You may be a good candidate for a nipple and areola correction if:
1. You’re bothered by the size, shape, or inversion of your nipples or areolas
2. Your nipples have changed after pregnancy or breastfeeding
3. You have congenital or developmental nipple asymmetry
4. You are not currently breastfeeding
5. You’re in good health and have realistic expectations

THE NIPPLE AND AREOLA PROCEDURE

Nipple and areola surgery is a relatively quick procedure, typically taking 1 to 2 hours, and is performed in-clinic under local anesthesia. You’ll be awake but comfortable throughout, and you’ll go home the same day with a light dressing and a simple aftercare plan to ensure a smooth recovery.

Enlarged Nipples:
If your nipples are disproportionately large in length or width, Dr. Parag Mahadik carefully trims the excess tissue while preserving the milk ducts and nerve endings to maintain sensation and functionality. The scar is strategically hidden at the base of the nipple, ensuring it blends seamlessly into your natural anatomy. Over time, the incision fades significantly, leaving minimal visible evidence of the procedure.

Inverted Nipples:
For mild to moderate cases (Grade 1 or 2), Dr. Mahadik releases the internal fibrous bands that are pulling the nipple inward. Fine sutures are used to support the new outward projection, ensuring a natural and lasting result. In more severe cases (Grade 3), where the milk ducts are shortened, partial division of the ducts may be necessary. While this can affect future breastfeeding, Dr. Parag Mahadik will discuss all options and risks during your consultation to help you make an informed decision.

Areola Reduction:
To reduce the size of enlarged areolas, a circular incision is made around the outer edge of the areola. Excess skin is carefully removed, and the incision is closed with dissolvable sutures. The scar is hidden along the natural border of the areola, making it virtually unnoticeable once healed. This technique restores proportion and creates a smoother, more balanced appearance.

Puffy Areola:
Puffy or dome-shaped areolas occur when underlying tissue protrudes, creating an uneven contour. Dr. Parag Mahadik uses a combination of liposuction with a micro-cannula to gently remove excess fat and fibrous tissue beneath the areola. This flattens the profile, giving the areola a smoother and more refined appearance.

The techniques used in these procedures are highly precise and designed to minimize trauma to surrounding tissues. Whether it’s preserving sensation, maintaining functionality, or ensuring scars are discreet, every detail is carefully considered. This is why, a professional of renowned Dr. Parag Mahadik’s caliber tailors each step of the process to your unique anatomy and aesthetic goals, ensuring results that feel natural and aligned with your vision.

These surgeries are minimally invasive and performed with your comfort and safety in mind. Dr. Parag Mahadik’s meticulous planning and attention to detail ensure that your results not only enhance your appearance but also boost your confidence. With proper aftercare, you’ll be back to your daily routine in no time, enjoying the subtle yet impactful changes these procedures can bring.

RECOVERY AND RISKS OF NIPPLE AND AREOLA SURGERY

Recovery timeline:

  • Day 1–3: You may experience mild discomfort, swelling, and tightness, but don’t worry—this is completely normal. Pain relief medication will help keep you comfortable during this time. Rest and avoid strenuous activities to allow your body to heal.
  • Day 4–7: to sponge baths only to keep the dressings dry and clean. This helps ensure proper healing and reduces the risk of infection. By the end of the first week, you’ll likely start feeling more like yourself.
  • Week 2: ’ll visit the clinic for a quick review. The sutures are dissolvable, so there’s no need for removal, and Dr. Mahadik will check that everything is healing as expected.
  • Week 4: Most patients can resume their full normal activities by this point, including light handling of the nipples. You’ll be able to return to work, exercise, and daily routines with confidence.
  • Month 2–3: will begin to fade and blend into your skin, becoming even less noticeable over time. Sensitivity in the treated area typically returns to its normal range, though this can vary from person to person.

While complications are rare, it’s important to be aware of the following:

  • 1. Bleeding, infection, or delayed wound healing: These risks are minimized with proper aftercare and our famous Sydney’s Cosmetic surgeon, Dr. Mahadik’s meticulous technique.
  • 2 .Loss of nipple or areola sensation (rare): Temporary changes in sensation are common, but permanent loss is extremely rare. Dr. Parag Mahadik takes great care to preserve nerve endings whenever possible.
  • 3. Impact on breastfeeding in severe inversion cases: If you have Grade 3 inverted nipples and milk ducts need to be partially divided, this may affect future breastfeeding. Dr. Mahadik will discuss this with you during your consultation to ensure you’re fully informed.
  • 4. Scar irregularities (usually minimal and well-hidden): Scars are carefully placed along natural borders to make them discreet. Most patients find they fade beautifully over time.

Dr. Parag Mahadik makes sure to use gentle, refined techniques to minimize risks and prioritize both your long-term comfort and aesthetic results. With his expertise and attention to detail, you can trust that your recovery will be as smooth and stress-free as possible.

Frequently Asked Questions

Scars are well hidden at the junction of areola and normal skin. They look pink and raised for the first 2 months but then fade over the next year. Scars at base of the nipple are forgiving given the natural rugosities in the area.
The surgical side is to be protected with padded bras over an occlusive dressing to avoid any handling or trauma. It is OK to handle them after 4 weeks.
It is very common for the areolas to enlarge in pregnancy. They seldom go back to normal. We recommend reduction surgery once you are done with childbirth. The procedure does not affect milk ducts in any way.
Yes and No. Only severe nipple inversions need release of the milk ducts where it may be partially affected. For the vast majority of cases milk ducts aren’t touched. Dr. Parag Mahadik will assess you and answer this specific question at consultation.
The nipples may become oversensitive in the early postoperative period. They will slowly get back to normal in about 3 months. In severe inversions ,they may partly lose sensation.
