What is Blepharoplasty?

Blepharoplasty, also known as eyelid surgery, is a surgical procedure that rejuvenates the upper and/or lower eyelids by removing excess skin, tightening muscles, and repositioning or removing fat. It targets The Distressed Look, characterized by puffiness, sagging, and drooping around the eyes that can make a person look tired, aged, or even obstruct their vision. The goal of the procedure is to restore a more youthful, alert, and refreshed appearance without altering the natural eye shape.

Upper eyelid blepharoplasty is commonly performed to correct drooping eyelids that create a ‘hooded’ look or interfere with vision. It involves removing excess skin, and in some cases, a small amount of muscle or fat. The incision is placed within the natural eyelid crease, making scars virtually invisible once healed. Patients often report looking more awake and rested after the procedure.

Lower eyelid blepharoplasty addresses puffiness, under-eye bags, and loose skin beneath the eyes. This can be performed either through a transconjunctival approach (from the inside of the eyelid) or externally just below the lash line. The procedure may involve fat repositioning, removal of excess skin, or muscle tightening. It helps smooth the under-eye area and enhances the youthful contour between the lower eyelid and cheek.

TYPES & VARIATIONS OF A EYELID SURGERY

1. Standard (Full) – This involves removal/ adjustment of all 3 components, viz, excess skin, loose muscle and excess/ baggy fat. It is a 1-hour procedure and recovery time is 3-6 days. The scar is hidden in the upper or lower eyelid skin crease.


2. Transconjunctival Blepharoplasty – The procedure is reserved for younger people and for lower blepharoplasties. The procedure is done from inside of your eyelid, so there is no scar on the skin.


3. Pinch Blepharoplasty – for early and minimal skin excess in which a thin strip of only skin is removed


4. Eyebags surgery – This is a type of lower eyelid surgery with increased complexity. In addition to all components of a standard blepharoplasty, it involves removal and redistribution of fat. The redistribution is essential to avoid under-eye hollowing from fat removal to create a smooth lid-cheek junction

AM I A GOOD CANDIDATE FOR BLEPHAROPLASTY

Blepharoplasty is a safe, high-satisfaction procedure suited to most healthy adults over 35 — but mental readiness and realistic expectations are just as important as physical suitability.
You may be a good candidate if you:

  • Have drooping upper lids or puffy lower lids
  • Are in good general health (e.g., well-controlled diabetes, no major heart issues)
  • Don’t smoke (or are willing to stop pre/post-op)

  • Have no serious eye conditions like glaucoma
  • Understand what the surgery can — and can’t — do

THE BLEPHAROPLASTY PROCEDURE

Depending on your level of comfort, blepharoplasty can be done under local anaesthesia with or without sedation. You will typically be discharged the same day of the procedure, which can take place in a clinic or hospital. Blepharoplasty comes in a variety of forms, and our Cosmetic surgeon at Esmee Clinics, Mumbai Cosmetic surgeon and Sydney’s aesthetic surgeon Dr. Mahadik, will customise the procedure to your unique facial structure and aesthetic preferences.


Upper eyelid surgery
1. A tiny incision is made inside the upper eyelid's natural crease.
2.Dr. Mahadik firstly tightens the muscles, then trims extra skin, and finally,removes or repositions fat that originally contributed to puffiness.
3.After this process, the incision is closed with fine sutures, hidden discreetly in the eyelid crease.
4.This procedure gives the eyes a more lifted, younger appearance, helps with vision improvement, and lessens eye strain, so you can be confident with your eyes.


Lower Eyelid Surgery
The incision may be made:
1.Slightly below the lower lash line (traditional approach), or
2.Inside the lower eyelid (transconjunctival approach — scarless).
- Next, protruding fat is swiftly removed or redistributed to eliminate the persistent under-eye bags.
- In some cases, Mumbai’s highly skilled and qualified Cosmetic surgeon Dr. Mahadik may seek to tighten the skin or the muscle to address laxity or “white show” (where the lower lid pulls away from the eye).
- A special technique may be used to blend the lid-cheek junction, ensuring a natural, smooth contour, and a better, graceful look.


Fat Repositioning vs. Removal
Dr. Mahadik concentrates on preserving natural volume by repositioning fat rather than simply removing it, which in turn, prevents a hollow or “overdone” look.


Time & Setting
1.Duration: 1 to 2 hours (longer if both upper and lower lids are done together).
2.Setting: In-room procedure under local anaesthesia for most patients. Sedation or general anaesthesia can be used if needed.
3.Discharge: Typically within 1 hour after the procedure is completed.

RECOVERY AND SIDE EFFECTS

  • Day 1–2: Use iced saline pads, rest with head elevated. Expect swelling and tightness.
  • Day 3–5: Most swelling peaks and starts reducing. Mild bruising. Light walking allowed.
  • Day 5–7: Sutures (if not dissolvable) are removed. Pain minimal. Vision may still be slightly blurry.
  • Day 7–10: Light makeup can be worn. Most patients return to work or social activity.
  • Week 2–3: Bruising subsides significantly. Residual swelling fades.
  • 4–8 weeks: Natural results begin to show. Scars soften and fade.
  • 3–6 months: Final results visible. Eyelid contour settles fully.

Rare side effects:

  • Infection or bleeding
  • Scarring (usually minimal and well-hidden)
  • Ectropion or entropion (inward/outward turning of eyelid)
  • Asymmetry (natural variation may exist between eyes)



Common side effects:

  • Temporary swelling and bruising (especially under the eyes)
  • Mild irritation, tightness, or dry eyes in the first few days
  • Temporary blurred vision or watery eyes
  • Mild discomfort or tenderness


PRO TIPS
Use sunglasses when outdoors to protect healing skin.
Avoid strenuous activity, eye strain, and alcohol for at least 2 weeks.
Lubricating eye drops may be prescribed to ease irritation.

“Recovery” means that there are no noticeable incision marks, swelling or bruising to the outward public. On average it takes 2 weeks. Blepharoplasty is not a painful procedure. Bruising and swelling (80-90%) settles in 2 weeks and work can be resumed.
Any surgery produces a scar. Upper eyelid surgery scar is very well hidden in the crease and heals quite well. The lower lid scar is just under the eyelashes and blends with your surrounding skin.
The gold standard for excess droopy skin is blepharoplasty. Some non or minimally invasive treatments can help select patients.
